Waiver Site Login Page
  If you have a college network ID, use the option for "Five College Students, Faculty, and Staff" at the top of the Login page. This link will allow you to select your college and enter your credentials. If you are a site manager or admin user, you will be redirected to your administrative page. All others will be redirected to the waiver form.

If you do not have a college network ID, and you want to complete a waiver, use the "To complete a waiver" link in the center of the page, and then follow the instructions on the page to request access to the waiver form.

Managers and administrative users who do not have a college network ID must be preregistered by Five Colleges staff. Please contact us if you need an account. Once your account is created, you can use the "Waiver site administrators and managers" link at the bottom of the login page. This link will also enable you to change your password, or to create a temporary password if you have forgotten it.
